This application
does not guarantee that I will be placed as an intern with
a company. This application only allows me to participate
in the process and to use the Aztec Business Alliance Resources.

It is my responsiblity
to look for internship opportunities available and determine
which opportunities I will apply for.

If I accept an internship
position, I have the option of receiving class credit for
Management 401 CR/NC or Business 401 CR/NC. It is my responsibility
to register and complete the requirements of each the course
and I am also responsible for submitting a completed contract
to the Internship Bureau office that must be signed by both
the employer and myself.

If I participate
in a Sponsored Internship I must complete the total hours
required by the Internship Bureau in order to receive the
stipend. I understand and agree that I will not get paid for
an incomplete Sponsored Internship.

If selected, I will
be expected to fully participate in the Sponsored Internship
Program. Failure to properly participate in the program will
result in termination of the internship status as an intern
and withholding of stipend.

I understand if I
am placed with a company through the Internship Bureau, it
is my responsibility to inform the Internship Bureau office
immediately.

I acknowledge that
participating in an internship may require local travel to
off-campus businesses and am aware of the risks involved while
driving and/or using public transportation.
